What counts as an injury designation?

The Premier League and FPL flag a player's fitness with a status (fit, doubtful, injured, or suspended) and a "chance of playing" percentage for the next round. A change to either, a new knock, a return to training, or a suspension, shows up here as soon as it's confirmed.

How chance-of-playing percentages work

75% or 50% usually signals a genuine fitness doubt worth watching before your deadline. 25% or 0% means a player is unlikely to feature or has been ruled out entirely. These percentages come directly from the club and are updated by FPL as new information comes in. It's not a prediction, it's the official estimate.
